This statement applies when you use the “Quick Online Assessment” form. By ticking the box before sending, you give explicit, informed and freely given consent to the processing below. Not consenting does not prevent you from booking; you can also book by phone or WhatsApp.

• Name, phone number and optional e-mail address
• The symptoms you select (e.g. heavy legs, swelling, visible veins)
• Your contact preference and optional message

This data is processed only to review your symptoms before an appointment, plan a suitable appointment and contact you. The form is not a diagnostic tool; diagnosis and treatment decisions are made after an examination.

The form content is sent by e-mail to the doctor over an encrypted connection via the hosting server; it is not stored on the site or in a database. It is not shared with third parties for advertising or marketing.

You can withdraw your consent at any time by writing to info@drcemalaslan.com. Withdrawal applies to processing after that date. See the Privacy Notice for details.
